Seeking Info About Kappa Free Light Chain Levels And Kappa/lambda Ratio

I simply want to know what a concerning level of kappa flc and k/l ratio are. (Phone number can only be seen by MyMyelomaTeam users)? Both of mine are high (lambda normal, obviously) but not yet to 100, and none of my doctors (oncologist, PCP) can tell me, they just say "watch and wait" (which I totally understand), and my oncologist said although a BMB is indicated for MGUS, they won't do a baseline bone marrow biopsy until my levels get a lot higher, my PCP said a BMB isn't standard?!

Hello Cheryl, I have Lambda light chin myeloma and my blood work is being done by the mayo clinic and my results this last week are Kappa Flc, S 0.7700 - Lambda Flc 0.3900 and Kapp/Lambda Flc, S 1.97. The Kappa is in the normal rang 0(Phone number can only be seen by the question and answer creators). The Lambda is in the low range 0(Phone number can only be seen by the question and answer creators) and the Kappa/Lambda is in the high range 0(Phone number can only be seen by the question and answer creators) but my oncologist said that because of medication I am taking the ratio in this range should be interpreted with caution an that I should not worry as I am doing great. Lets hope that it stays that way. I hope that this information is helpful. Best wishes. 👨‍✈️

@A MyMyelomaTeam Member Excellent idea, I'll check into that! I have checked into 2nd opinions via online with a few other clinics (Cleveland Clinic, Dana-Farber), but they were way out of my price range since insurance didn't cover them. I'll check with this MM specialist in Dallas I've been looking into, he is on my insurance and possibly comes to a satellite clinic closer to my home, if not, maybe he does Zoom! You are well covered with all the blood testing but I completely understand the peace of mind you are looking for with a BMB.

Not all MM is lambda/kappa free light chain. Do you have elevated Immunoglobulins (IgA, IgG, etc) and M-spike (from SPEP test)?
We have all heard of stories about "if I just got this test sooner". So I think you should concentrate your research on how to get a test performed when none of your doctors want to do it. It will probably require a referral to another doctor (maybe out of town/state) with out the concent of your current doctor's.
I just don't know why they do not want to order a BMB. You need to figure out how to manipulate the doctors and the system to get it. Keep searching for advice.

Thank you @A MyMyelomaTeam Member! I do understand not all MM is K/L FLC, it's just that my MGUS is classified as that (elevated kappa), low m protein, slightly elevated IgG.

I get mass spec testing, along with spep and IFE every 6 months. I guess the doctors feel that is enough given my low/high levels of lab results. But I do agree with you, I will keep searching.
