Hi, my brother Lance has 6 compression fractures in his spine at the moment, he was diagnosed in Apr./23. He also had something on his one knee, had radiation for that and hasn't said anything about it since. He has had physio for his back, that is all so far. Being special needs adult, he doesn't register the pain the same way I would, so we did have to help him learn to really tell us when he is in pain, then has a hydromorphone pill and an extra strength Tyl, if we are out with him - at home, he only has the hydromorphone at night..we, however, would be having more than that, for sure, with the fractures. He hasn't had problems with the kidneys yet. He also has 20 mg. Dexa each Fri., has Daratumumab once a month now and Lenalodomide (not sure of sp.) 21 days at a time. Best wishes for you!

Certainly! Let's discuss the treatment options for kidney and bone issues due to multiple myeloma, especially considering your current medications: Daratumumab, Bortezomib, and Dexamethasone.

Kidney Issues:
- Treatment Focus: Addressing the underlying myeloma is crucial. When myeloma is controlled, kidney function often improves.
- Medications:
- Dexamethasone: High-dose dexamethasone can aid in kidney function recovery.
- Bortezomib: Often prescribed for those with renal impairment as it helps manage protein levels in myeloma cells.
- Supportive Care: Adequate hydration and correction of increased calcium and uric acid levels are essential.

Bone Issues:
- Medications:
- Daratumumab: This monoclonal antibody helps target and kill myeloma cells, potentially reducing bone damage.
- Bortezomib: Also helps in managing bone health by controlling myeloma cell proliferation.
- Additional Therapies: Bisphosphonates or other bone-strengthening treatments may be recommended to manage bone health.

Monitoring and Side Effects:
- Kidney Function: Regular monitoring is necessary as medications can affect renal function.
- Cardiovascular Risks: Be aware of potential cardiovascular complications, especially with corticosteroids like dexamethasone.

It's important to stay in close contact with your healthcare team to manage these treatments effectively.

I also had kidney problems with myeloma. My nephrologist ordered plasma exchanges which I am sure helped save my kidneys. These took place before my sct while I was having chemo treatments. The myeloma treatments and sct also improved my kidney function. That all said I am 3 years out from my sct. My kidneys - while damaged - are stable. My GFR has stayed between 38-43 and I see my nephrologist every 4 mths.

I am taking Dexamethasone 20mg once in a week along with Daratumumab and Bortizomib. Is it sufficient for kidney recovery? How to protect kidney degradation or further failure. How much dexa should be taken for kidney recovery? My current creatinine level fluctuates between 3.4 to 6.5. Please guide. Rgds Abhinay
