How Efficient Are The Results From A 24 Hour Urine Test In Detecting Multiple Myeloma Cells?

My oncologist scheduled me in March 3 months earlier to monitor a .3 m protein that was found in my urine
I did not throw away the 1st pee like it said I included it in the collection plus I may have been dehydrated somewhat as I didn’t eat watermelon and drink pedialyte plus I’ve exercised way more than my normal with potty training this new puppy my husband got. He has to go outside to pee or poop every 4 hours
So many factors can be causing the slight increase in m proteins found in my 24… read more

Hi Angie - I'm finding there's plenty of ways to have a low MSpike. For myself, I don't even bother urine MSpike testing. I've had 2 urine tests since I started 3 years ago. I do continue with Monthly Labs and Quarterly blood sample clonoSEQ tests and No BMB unless I report as Positive.

While I'm Negative at 10 -6 from a blood sample clonoSEQ, I then check my Kappa/Lambda & Ratio, my Bun, Bun Ratio, Create and eGFR/eGFRNA (these 4 relate to the health of the kidneys), then my ALT/AST (liver health), and then all of the Inflammation markers of B2M, CRP, LDH, TSH. Next my Ig levels of IgG, IgA, IgM. Also, my Glucose and Vit D. All components of my CBCs and CMPs (which contain a few of the above).

When all of these are Good, to get concerned about one isolated item, though it's important to know about and track, it's definitely nothing to get worried about.

I had an inadvertent issue with my TSH (thyroid) going slightly above normal and then a tiny MSpike of 0.02 and the next month of 0.04, even though my blood sample clonoSEQ reported 10 -6 Negative. My Onc was a bit concerned of a potential thyroid issue. I went and reviewed my older Labs and found 10 months prior I had an Iodine test that was high. I was, on a daily basis, getting far too much Iodine from my Daily Vit and large seaweed sheets (I was eating plenty throughout the week). As soon as I stopped eating the seaweed sheets, my next month's MSpike was gone and my TSH returned to normal. I've had two Monthly Labs since and everything remains Normal.

Two years ago, my sister had a EvuSheld Shot that gave her a False MSpike that went away on its own (nothing to do with MM). People get Vaccines, Flu Shots, and Covid Shots that all introduce False Positives.

I've learnt to Look at the Whole Labs Picture and realize that for all of these Interconnected Pieces to be So Very Good, there's no way one little item is a serious issue. It should be tracked and made sense of but definitely it needs to be considered against everything going on.

My priority is the clonoSEQ, then everything else as a Whole. Since I get Full Monthly Labs, I get to see little meaningless blips that most people never realize are going on under-the-hood.

A 24-hour urine test is a valuable tool for detecting abnormal proteins, like M proteins (or Bence-Jones proteins), which are associated with multiple myeloma. However, factors like dehydration, improper collection, or increased physical activity can potentially affect results. Since you included the first urine and may have been dehydrated, it’s worth discussing these details with your oncologist. They may recommend repeating the test for more accurate monitoring.

I feel better now and realize how many parts play in determining the whole picture!
So much to learn just how complicated this mm really is and all the intricate details!!
And some people say there is no God? All this is so amazing and so much yet to know!!!
