For further expansion on the above answer you should look up the 20-2-20 test. Bone marrow cells above 20, lamba/kappa ratio above 20 and m protein above 2g/dl. If bone cells are over 10% you are then smoldering.

Medical professionals assess the risk of MGUS progressing to multiple myeloma (MM) using factors like:

1. M protein (M spike) level: Higher levels increase risk
2. Immunoglobulin type: Non-IgG MGUS (e.g., IgA or IgM) has a higher risk.
3. Free light chain (FLC) ratio: An abnormal ratio suggests higher risk.
4. Bone marrow Show Full Answer

4. Bone marrow plasma cells: Higher percentages indicate greater risk.

These factors help classify MGUS as low, intermediate, or high risk. Regular monitoring is crucial.
