What Is A "good" Or Manageable M Spike?
Is there such a thing as a good or manageable M Spike? Or is it different for each type of Myeloma? My M Spike was down as low as .15 back a few years ago. It now has been hovering around .40 since I changed therapies. I started with Revlimid, Velcade, Dex on 2018. Now on Darzalex Fastpro, Kyprolis, Dex. I know I want it lower but my doctor says it may never go lower. Maybe just hanging out at .4 is ok.

After my stem cell transplant my m spike was 0.2. I take 10mg Revlamid daily as maintenance. It stayed at 0.2 for a year and has since been generally increasing, but bouncing around. Highest was 0.5, but it's now back down to 0.4 for last 3 months. Doctor says no change in treatment until mine reaches 1.0.
Steveh, a normal m spike is 0.0. But, if your m spike is stable. You can be fine for many years as long your treatment is working and your other labs and imaging are good. I had a m spike for 2 years after my transplant. It finally went to 0. I still get one from time to time. Continue with your treatments and monitoring. Also Darzalex produces a slight m spike on the kappa region on an electrophoresis. Most labs have the Daratumab specific IFE and can tell the difference.
DarzalexFasPro can produce a false MSpike as high as 0.2
The Testing Lab needs to perform their Lab Test in a special manner to accurately cancel this error. Originally, my tests were not being done correctly. I had my oncology office request corrected testing and the Lab was able to do so.
The best MSpike is Not Detected. At 0.5 or less one of my Docs said there’s nothing they’d do treatment wise. Other Docs might have a different opinion.
