Contact Bristol Myers Squibb, they will send a application that you and your oncologist fills out your mm team should pass that to the hospital pharmacy and work on getting you help with the cost.
I am on MC Bronze and I have BluemedicareRx. With that coverage it pays for my monthly Darzalex injection and 90% of the Revlimid. I have a grant that picks up the balance (my copay). I have not paid a penny for any of my treatments. My copays for other meds (dexamethasone, acyclovir, compazine) are minimal.
Do you think you need GAP? I have Medicare A, B, C and D which covers my prescriptions. I think you’re going to have out of pocket expenses anyway, I would talk to a rep. to see what’s available and what’s good for you. I love what I have, but anything I think you’re going to have out-of-pocket.
